Two Ways to Adjust Nipple Direction with Breast Implants

In the breast circumstances, nipples project straight forward. But what if one or both of the nipples point inward (cross-eyed) or aim outward (wall-eyed)? Let’s eyeball a few ways to straighten the aim of the nipples with breast implants.

Using a 3D image of an actual Changes Plastic Surgery patient, we’ll demonstrate how the placement of a breast implant can alter the direction the nipples point.

Let’s start with a view of our patient’s true anatomy without implants. Notice a slight projection inward of the nipples (cross-eyed) with narrow placement of the breasts on the chest wall.

Placing breast implants outward in the pocket so the breasts appear better balanced on the chest only pushes the nipples farther toward the center of the body exaggerating the nipples’ cross-eyed appearance.

Finally, moving the implants inward in the pocket allows the nipples to point more outward more, but creates an unnatural cleavage, exaggerates the narrow breast width and creates a uni-boob appearance.

Dr. Lee performed breast augmentation on this patient using 350 cc. silicone implants in the right breast and a 375 cc. silicone implant in the left breast to correct asymmetry in size. To improve the nipple direction, he created the subpectoral (below the muscle) pocket slightly more medially than laterally in order to prevent exaggerating the natural “cross-eyed” nipple placement.  The results are superb.

It is important to note that natural anatomy cannot be altered with breast implants, but the direction of the nipple can be somewhat corrected with slight adjustments in implant placement.
